FUNDING

• A $1000.00 scholarship will be awarded to a High School Senior.

CRITERIA

• Minimum of a 3.5 GPA, with copy of transcript.
• The applicant must have the name of the college or university they are attending.
• The applicant must be a previous or current patient of Dr. Iverson.
• Student must create a Video Titled: "How a Smile can change the World". Guidelines

SELECTION COMMITTEE

• High School Principal or designee
• Two High School Teachers
• One member of Dr. Iverson's Team
